This notice sets out information from staff members to prove they are eligible to take shared parental leave and claim shared parental pay following the birth of their child. It must be completed and submitted to you at least eight weeks before leave is due to begin.
This notice can be provided to a staff member for completion where either the staff member, or the person with whom they will be sharing the shared parental leave, will be giving birth to the child.
The parts of the notice that your staff member must submit depend upon their relationship with the child. The questionnaire will enable you to generate a notice containing the relevant sections, which you can then give to your staff member for completion.

You should give this letter to your staff member when they notify you that they intend to take shared parental leave following the birth of their child.
